Balanced Thoughts Therapy is a small but growing group practice looking to add another therapist to our clinical team. Balanced Thoughts Therapy is an insurance-based private practice operating in Maryland with in-person office space available for clinician use in the Rockville/Derwood area.

Position Overview

We are seeking a Licensed Social Worker (LCSW/LCSW-C) or Licensed Professional Counselor (LCPC) to join our growing clinical team.

In this role, you will have clinical autonomy to manage your schedule without caseload minimums, select clients who fit your clinical focus, and determine your therapeutic approach. We provide full administrative support, including marketing directory placements, insurance credentialing, client billing, a practice laptop, and Electronic Health Record (EHR) software. This practice specializes in treating anxiety disorders, mood disorders, and trauma disorders.

Key Responsibilities & Duties

1. Clinical Care & Patient Management

• Provide high-quality outpatient mental health services, including Diagnostic Interviews and Individual Therapy. Family Therapy, and Couples Therapy are not a primary clinical focus of this practice, but services can be delivered at clinician discretion.
• Exercise complete clinical discretion regarding client diagnosis, therapeutic interventions, treatment frequency, discharge planning, care coordination, and outside referrals.
• Self-select clients who align with your clinical expertise (caseload capped at 33 sessions/week).
• Participate in client onboarding and follow up on practice-facilitated client inquiries.
• Maintain strict adherence to state/federal laws, HIPAA compliance, and professional ethical standards.

2. Clinical Documentation & Administration

• Maintain complete and timely patient records in the practice EHR system.
• Complete documentation according to practice timelines:

- Session Occurrence & Billing Codes: Logged by the end of each business day.
- Diagnostic Interviews & Initial Treatment Plans: Completed by the end of the next business day.
- Progress Notes & Assessments: Completed within 2 weeks of the date of service.
- Treatment Plan Reviews: Updated at least every 180 days.

- Discharge Summaries: Completed within 2 weeks of the final session

Qualifications & Requirements

• Active, unencumbered professional clinical license in the State of Maryland (e.g., LCSW-C, LCPC).
• Have experience with independent treatment planning and clinical decision making.
• Ability to maintain timely and insurance-compliant documentation (templates and direction will be given).
• Ability to be responsive to both clients and practice needs/inquiries. Response time to client needs and inquiries is expected to be within 48 hours.
• Active Professional Liability Insurance ($1M per occurrence / $3M aggregate) naming Balanced Thoughts Therapy as a certificate holder.

Compensation Structure

Compensation is based primarily off of service codes. For example, a 90837 is a 53-60 minute therapy session. You would be reimbursed for the clinical hour, which includes the psychotherapy service and documentation. Reimbursement rates for service codes for insurance-based clients include the following:

• Insurance / Session Pay (Commission-Based): Paid bi-weekly per completed clinical hour based on service code:

- 90791: $72 per diagnostic psychosocial assessment

- 90837: $65 per 53-60 minute psychotherapy session

- 90847: $54 per family therapy session

• Private Pay Clients: Earn 70% of collected private pay rates. The practice has advertised private pay rates of $225 for 90871 and $175 for 90837, but therapists are allowed to negotiate private pay rates with their clients via sliding scale (minimum rate set at $100/session).
• Start-Up / Onboarding Pay: Provide 15 hours at $20/hour to complete initial setup, directory profile creation, paperwork, and training.
• Administrative Pay: Two hours of administrative time is compensated per week at a rate of $20/hour for every 10 clients. This time is for the completion of any case coordination, treatment planning, or note completion needed.

We are hiring for supervisors who are eligible to become or already are board approved supervisors in Maryland with either the Board of Social Work or Board of Professional Counselors. Our clinical therapists provide outpatient, individual, family, and group therapy to clients of Trauma Specialists of Maryland, LLC with locations in Downtown Frederick, Urbana, and Ballenger Creek, MD.  Clinical therapy services include treatment of adults, adolescents, couples and families. We are an EMDR focused practice and train all of our clinicians in EMDR if you are not trained already.  Other treatment modalities such as IFS, DBT, Neurofeedback are also encouraged.
